The Case Management Services Coordinator is responsible for overseeing case management, supervising the case management team as well as providing case management services to CHGA youth and, if necessary, families as they pursue reunification. This is a leadership role and provides support to all programs in carrying out the day-to-day responsibilities in order to fulfill the mission of Covenant House Georgia. The Case Management Services Coordinator reports directly to the Director of Residential and Case Management Services.

ESSENTIAL RESPONSIBILITIES: Covenant House Georgia reserves the right to unilaterally modify, remove, or add to any of the following:
. Oversee Case Management and supervise case managers, AmeriCorps Service members, and interns.
. Network with community resources and build referral network.
. Develop and provide leadership and training to case managers.
. Monitor data for program outcomes and performance and utilize data to drive service provision
. Complete a monthly report
. Document individual and group notes on youth receiving case management services.
. Ensure timely and accurate collection and documentation of data relating to program services by completing monthly audit of the case records.
. Ensure compliance of files in accordance to specific City, State and Federal guidelines.
. Lead weekly case review
. Participate in other CHGA programs and activities as required.
. Perform all other tasks as assigned by CHGA administration.
. Monitor and evaluate program effectiveness and discuss observations with supervisor to maintain and enhance program functioning.
. Uphold Covenant House Georgia standards of conduct, policies and procedures at all times.
. Attend and participate in all scheduled or mandatory meetings, training and supervision sessions as requested.
. Model appropriate, professional behavior for staff to establish and maintain superior services to youth in a safe and inviting atmosphere.
. Ensure all programs are safe and free from any conditions that may cause harm or danger to youth, staff, volunteers, or visitors.
Case Management Duties:
. Oversee case management services to CHGA youth. Help all staff develop positive relationships with CHGA youth. Effectively manage and ensure the following:
. Case Assignment: Ensure youth are assigned a case manager within 24 hours of entering the crisis shelter.
. Expanded Assessment and Case Plan: Ensure Case Managers complete the expanded assessment within 72 hours of case assignment and ensure the case plan is completed within 7 days of assignment
. Documentation: Case managers are expected to make weekly progress notes in both ETO and Client Track databases. Ensure Case Managers are reviewing transition reports, medication logs, critical incident reports, and any pertinent documentation.
. Case review: Case Managers are required to maintain documentation and be prepared to present cases at regularly scheduled case reviews.
. Group Facilitator: Case Managers will facilitate therapeutic and/or psychoeducational group activities with the youth on a regular basis. Case Managers are also responsible for facilitating youth orientation
. Individual Contact: Case Managers are required to meet with each youth on their caseload weekly to drive services as indicated in the case plans.
Knowledge and Abilities:
. Knowledge of service providers in Atlanta metropolitan area
. Ability to:
. Teach: interviewing techniques for creating rapport and building relationships; crisis intervention techniques for working with youth who are homeless, runaway, or at-risk of being exploited; goal setting, case planning, implementation, follow up and termination
. Establish and maintain an effective working relationship with co-workers, supervisors, representatives of other agencies, and the public.
. Effectively communicate, verbally and in writing, with both internal and external individuals and/or businesses.
. Respond in a timely manner to emergencies and resolve issues with client and/or staff.
. Defuse physical and verbal confrontations with youth using appropriate techniques and/or interventions.
. Effectively manage organizational change and work as part of team to implement improvement measures.
. Pivot and work in a fast paced environment
. Knowledge of cycles of poverty, and other risk factors impacting youth served (substance abuse, sexual exploitation, mental and physical health)
SUPERVISORY DUTIES: This position supervises Case managers, AmeriCorps service members and interns. This position is responsible for managing staff schedules and approving staff’s time.
EDUCATION: Master’s degree in Social Work or equivalent social science field required; LCSW Preferred
EXPERIENCE: 5 years’ experience providing case management and 2 years’ experience supervising employees required.
WORK ENVIRONMENT/PHYSICAL DEMANDS:
. work a flexible schedule including evening hours is a requirement for this role.
