This position is dedicated to Rapid Rehousing programming. Essential functions of this role include core case management activities and stability services. The Case Manager will work with assigned residents both during and after housing placement with a focus on community support and home-based visits. This position requires a strong ability to initiate, engage, and build rapport with youth to support their independent living skills and housing permanency. This position is committed to providing individualized care that supports youth’s goal attainment. This occurs through assessment, planning, coordination, implementation, and evaluation of individual needs. Knowledge of Coordinated Entry developments/processes, Trauma-Informed Care, Housing First principles, harm reduction strategies, and Rapid Rehousing interventions preferred. Must be able to maintain a flexible schedule, including some evening and weekend work. All duties should be performed within the framework of the policies and procedures of Covenant House Georgia.
Reports to: Rapid Re-Housing Coordinator
ESSENTIAL RESPONSIBILITIES: Covenant House Georgia reserves the right to unilaterally modify, remove, or add to any of the following:
. Maintain a caseload of 10-20 youth aged 18-24 years old
. Conduct intake assessments with participants referred from the City of Atlanta Continuum of Care Coordinated Entry to assess housing needs and preferences.
. Verify youth eligibility for RRH services
. Identify housing resources; recruit and maintain positive relations with landlords; assist youth with completing applications, submitting documentation and lease execution
. In collaboration with the participant, develop individualized service plan with measurable goals and objectives based on the needs identified in on-going assessments to include housing stabilization and components of the financial plan, retention goals and employment goals
. Ensure that service plans account for participant preferences, choices and goals
. Set frequency and location of case management meetings based on individual participant needs and to accommodate participant
. Assist with move-in to unit, utility activation, and obtaining household items such as bedroom furniture, mattresses and housewares
. Educate participants on lease requirements and prohibitions, process for making monthly rent payments, tenant responsibilities such as maintenance of unit, good care of property.
. Collaborate with CHGA employment assistance team to secure employment and develop path to increase income
. Connect participant to CHGA services for behavioral and physical health or with external services and other resources.
. Continue to update service plan to reflect progress and address challenges
. Monitor housing stability and advocate on behalf of the participant with landlords to overcome any issues or challenges to maintaining suitable housing; provide crisis intervention
. Access need for financial intervention to prevent eviction
. Prepare for and participate in weekly case reviews
. Develop comprehensive transition plans for participants ready for independent living.
. Develop knowledge of the current community resources and referrals, including attendance at off-site meetings and trainings
. Conduct aftercare with participants for up to 12 months following exit
. Act as Primary contact and Housing Provider in ClientTrack Coordinated Entry for the City of Atlanta.
. Monitor and document regular progress of youth’s case plan and maintain case management files in both ClientTrack (HMIS), Efforts to Outcomes according to Data Integrity Plans.
. Timely respond to all requests for data correction and completeness
. Work with RRH team to monitor program outputs and ensure that program outcomes are met
. Uphold Covenant House Georgia standards of conduct, policies and procedures at all times.
. Attend and participate in all scheduled or mandatory meetings, training and supervision sessions as requested.
. Model appropriate, professional behavior for staff to establish and maintain superior services to youth in a safe and inviting atmosphere.
. Ensure all programs are safe and free from any conditions that may cause harm or danger to youth, staff, volunteers, or visitors.
SUPERVISORY DUTIES: There are no supervisory duties for this position.
EDUCATION: Bachelor’s degree in Social Work or related field required, MSW preferred
EXPERIENCE: 2-3 years relevant experience providing case management preferred
WORK ENVIRONMENT/PHYSICAL DEMANDS:
